COCODA Tanzania is a non-governmental organization established in 2000. COCODA’s establishment as its name suggests, resulted from intensifying concern among community members in Tanzania over the surging number of orphans, mainly resulting from the increased number of people infected by HIV and dying from AIDS, the majority being adults leaving behind young children without parents. This trend had a negative impact on social and economic development.

Hence COCODA Tanzania envisioned offering an effective response to the situation by providing education to the local communities on HIV/ AIDS, supporting orphans and most vulnerable children, providing vocational skills to youth, and establishing small-scale incomegenerating groups for a community sustainable economy.

Meeting Targets and Maintaining Epidemic Control (EpiC) is a global project funded by the U.S. President’s Emergency Plan for AIDS Relief (PEPFAR) and the U.S. Agency for International Development (USAID) which is dedicated to achieving and maintaining HIV epidemic control. FHI360 is the prime recipient of this funding in Tanzania.

COCODA is subcontracted by FHI360 to implement the EpiC project in Njombe and Mtwara regions. In the Njombe region, the project is implemented by COCODA in 6 councils (Njombe TC, Njombe DC, Makambako TC, Wanging’ombe DC, Ludewa DC, and Makete DC), and the Mtwara region the project will be implemented in all 9 councils (Mtwara MC, Mtwara DC, Masasi TC, Masasi DC, Nanyumbu DC, Nanyamba TC, Tandahimba DC, Newala DC, and Newala TC) to address critical gaps and bottlenecks in the response to HIV among key and vulnerable populations (KVP) and adolescent girls and young women (AGYW). The EpiC project works closely with the Government of Tanzania (GoT) at national and sub-national levels to deliver HIV and other services tailored to the needs of these groups.

Community Engagement Officer (CEO): 1 Position
Location
: Njombe Region
Reports to: Deputy Project Manager
Position Description
The Community Engagement Officer will work with the EpiC Regional Technical team to implement an HIV prevention program targeting the population at risk in the region. Working with Vulnerable populations including Adolescent Girls Young women, and other at-risk Populations. s/he will monitor the implementation of the SBC, and structural interventions in compliance with the overall regional work plan.

S/he will play a key role in ensuring that the SBC, structural activities are conducted according to the National standards and EpiC standard operating procedures, and that beneficiaries receive quality services; s/he will also play a crucial role in supervising and building the capacity of Peer educators, Peer mobilizers and Peer Navigators of key and vulnerable population during outreach activities (mobile testing and moonlight approaches) for HTS provision. s/he will be managing the implementation of Social Network Strategy (SNS) including Enhanced peer Outreach Approaches (EPOA) and Risk network referrals (RnR), S/he will be facilitating peer outreach and Peer navigators through conducting progress reviewing weekly/ monthly basis with PE, PN and PM. She/he will be working with Linkage and the case management team with health facilities and the community for a continuum of care. The Community Engagement Officer will oversee the implementation of online outreach strategies to expand program reach. He/she will support Adolescent and youth Economic strengthening by supporting the establishment of incomegenerating activities.

Job Details
  • Oversee and manage a team of peer educators, mobilizers, and navigators to deliver comprehensive HIV services including HTS, PrEP, HIVST, TB, GBV, and family planning.
  • Ensure effective implementation of community-based activities, including mobile testing, self-testing, index testing, and other outreach services.
  • Facilitate community access to condoms and PrEP services among key and vulnerable populations.
  • Conduct SBC training for adolescent girls and young women, as well as other at-risk population
  • Supporting Peer (educators, mobilizers, navigators) during Community works including demand creation, group education, individual education, outreach services for bot moonlight and mobile testing, and client referrals and linkages
  • Supervising and monitoring community activities including SBC activities to ensure Peers (educators, mobilizers, navigators) have all required tools, etc.
  • Coordinate and support the Peers (educators, mobilizers, navigators) to implement and monitor the timely implementation of comprehensive HIV services and provision of friendly services to all KVP clients
  • Manage peer educators and peer navigators to ensure proper planning and implementation of community engagement-related activities for demand creation, group, and individual education sessions
  • Support PEs to ensure that the vulnerability index tool is administered and filed for all AGYW enrolled in the project.
  • Facilitate the formation of economic strengthening groups among adolescents and youth
  • Support the establishment of a case management team in conjunction with the health facility
  • Work with project and district officials to facilitate community engagement training to, peer educators and navigators and other relevant training required

B: Networking
  • Build and maintain strong relationships with stakeholders such as implementing partners and LGAs
  • Identify and link the target population to relevant support services including Economic Strengthening and other care referral
  • Collaborate with other implementing partners to optimize service delivery.

C: Monitoring, Evaluation, Learning, and Reporting
  • Prepare monthly and weekly route plans for the project to reach the targeted population. Prepare document experiences, briefs, and success stories
  • Monitor Peers (educators, mobilizers, navigators)and Providers implement plans and progress of community engagement portfolio as per project targets
  • Provide weekly updates and monthly reports to the Program Manager.
  • Conducting weekly performances for Peer (educators, mobilizers, navigators) to assess performance and share areas of improvement
  • Working closely with the Data Manager to evaluate data and program progress.
  • Conducting monthly meetings with Providers and peers (educators, mobilizers, navigators) to share progress in a specific month and plan for the next month

Required Qualifications:
  • Bachelor’ degree in public health, social work, Community Development, Sociology, or other related degree required, 3 to 5 years of relevant experience working with KVP at the community level.
  • Knowledge of health and development programs in Tanzania including familiarity with the national KVP, youth and gender guidelines, standards, and protocols.
  • Experience in programs serving KVP and addressing gender norms, demonstrated cultural sensitivity and sound understanding of the needs of KVP and other vulnerable groups.
  • Experience in working with LGA, NGOs, CBOs, and health facilities to implement public health projects, especially those working with vulnerable populations.
  • Ability to manage tight deadlines and deliver high volumes of work with minimal supervision.
  • Experience in conducting trainings and workshops.
  • Experience in working with youth and adolescents.
  • High degree of proficiency in written and spoken English communication.
  • Well-developed computer skills.

Linkage and Case Management Officer – 1 Position

Reports to: Deputy Project Manager
Position Description:
Linkage and Case Management Officer is responsible for identifying, enrolling, and supporting individuals living with HIV in care and treatment. provides comprehensive case management, including adherence support, counseling, and community outreach. LCM will collaborate with healthcare providers and facility partners to optimize service and improve patient outcomes.

The Linkage and Case Management Officer will liaise with the Community engagement technical officer and biomedical officers to implement HIV prevention program targeting populations at risk in the region. LCM will work with a team to reach, serve, and maintain Key and Vulnerable populations, monitor case linkages during pre and post-counseling, and ensure HIV-positive diagnosed KVP individuals are linked to care and treatment centers. The LCM will also oversee community ART initiation, and retention, and ensure clients who miss appointments are brought back into care.

The LCM will be responsible for establishing and maintaining navigation processes according to Care and treatment standards and ensuring a client-centered approach is observed during service provision to KVP-positive clients. LCM will link KVP HIV-positive clients to economic strengthening activities and manage gender-related issues. Additionally, He/she will supervise and build the capacity of Peer navigators through progress review meetings and coordinate the case management team with health facilities and the community for a continuum of care.

Linkage and Case Management Officer will monitor Gender-based/Intimate partner Violence cases, including the status of referrals and linkages. LCM will play a crucial role in ensuring the well-being and support of individuals living with HIV and strive to improve patient outcomes through comprehensive care management and community outreach.

  • Provide technical support to Peer Navigators to reduce quarterly net loss by 95% and document best practices.
  • Work with peer navigators, healthcare workers, and facility partners to ensure 100% biometric registration coverage among all clients receiving care and treatment.
  • Collaborate with facility partners to achieve a 95% linkage to care rate for newly diagnosed clients in both community and facility.
  • Work closely and support healthcare providers in delivering ART adherence counseling and enhanced counseling for PLHIV with high viral load.
  • Mentor and support peer navigators and community-based health care workers in linking PLHIV diagnosed in the community to care and ensuring proper documentation.
  • Provide intensive adherence support to patients with Advanced HIV Disease (AHD) to improve treatment outcomes.
  • Assist healthcare providers and Peer navigators in early identification of missed appointments.
  • Conduct close follow-up on tracked missing clients who have promised to return to care.
  • Promote the use of national follow-up tools and ensure their proper documentation.
  • Facilitate regular meetings with Peer navigators, healthcare workers, and other IPs to share challenges and develop improvement strategies.
  • Responsible for identifying, enrolling, and supporting clients in Community ART, optimizing adherence for clients in Community ART.
  • Daily follow-up on case management by the team, from each service delivery point in close relationship with the Biomedical field officer
  • Reconcile and compile data into the regional tracking register and the regional partner listing and outcome register, and maintain it weekly
  • Conduct sites visits for at least 70% of the time
  • Working with the data manager/M&E Officer to report tracking and partner listing and outcome data to the MER team in the region using referrals from stubs
  • Prepare weekly and monthly reports related to tracking services and partner notification and success stories, as needed
  • Participate in meetings at the CSO level and related CHMT meetings

  • Perform other duties as assigned by the line manager

Requirements: Education, Work Experience, and Skills
  • Bachelor’s degree in medicine, Nursing, or related field with at least three years of relevant experience.
  • Must have experience in HIV/AIDS care and treatment programs. · Proven track record in case management experience
  • Strong report writing, planning, and presentation skills.
  • Experience working with health management teams is advantageous.
  • Prior experience in working with literate and non-literate audiences; experience in participatory methods
  • Experience in the KVP program is an added advantage
  • Experience in supervising a field team
  • Excellent interpersonal, communication, and organizational skills. · Proficiency in English and Kiswahili.

Biomedical Technical Officer (BTO) – 1 Position
Location
: Njombe Region
Reports to: Deputy Program Manager
Position Description:
The Biomedical Technical Officer (BTO) will be responsible for coordinating the implementation of EPIC Project biomedical services to the assigned council. He/she will work closely with the EpiC Central team, RHMT, and CHMT to ensure the provision of high-quality biomedical services including HIVST, PrEP, HTS, Family planning, Screening, and referral for
STIs, Gender-Based Violence, and TB. Also, in coordinating linkages and adherence support for HIV care and treatment to clients in their assigned counsel

Roles and Responsibilities:
Planning:
  • To participate in the Comprehensive Council Health Plans (CCHP) meetings and ensure EPIC activities are included in the respective CCHP.
  • Liaise with Community Engagement Officers to map hotspots and develop a directory of all hotspots where beneficiaries are found at the council level.
  • Conduct weekly review of the list of index sources developed by service providers and ensure all the providers conduct ethical and safe index testing services
Coordination and Supervision:
  • With support from the Project Manager, he/she will be responsible for liaising with the DMO office to secure Government biomedical providers who will provide biomedical services at the community level
  • Actively participate in the development of monthly operation plans in close collaboration with community engagement officers for targeted and focused HTS outreaches to the mapped hotspot areas and reach KVPs
  • Assist in organizing training/orientation for biomedical providers to be engaged in EPIC monthly operation plans and maintain the list of Service Providers working with EPIC project.
  • Monitor and coordinate community-facility referral and linkages services for health-related services including care and treatment, Family planning, STI & TB screening and treatment, and GBV services
  • Liaise with DACC and the DMO office to obtain commodities, supplies, and government M&E Tools for Biomedical services.
  • Supervise biomedical providers and ensure monthly planned activities are implemented and all Service Providers are reaching their allotted targets with fidelity.
  • Provide technical support and mentorship to the biomedical services providers within the assigned districts to ensure they meet standard of performance as per the government guidelines and project SOPs
  • Work closely with the CHMT members in performance monitoring, joint supportive field supervision at the site/council level, and ensure the outcomes of the visits are documented and recommendations implemented.
  • Jointly work with facility implementing partners at the council level to develop a sound Bi-directional referral network
  • Ensure all service providers properly fill all required National and Project M&E tools and timely submission of service reports appropriately, including monthly summary reports to the catchment health facilities.
  • Develop and timely share daily/weekly updates and monthly reports for all biomedical services.
  • Conduct monthly stocktaking and prepare reports on biomedical supplies and commodities. · Ensure biomedical supplies and other supportive commodities are readily available. · Collaborate with Case Managers and Peer Navigators to guarantee timely linkage and continuity of care and treatment services.
  • Partner with Case Managers and Peer Navigators to ensure timely baseline CD4 and CrAG testing for all eligible clients, with prompt reporting.
  • Ensure that all eligible clients undergo VL tests, with results reported accordingly.
  • Work with peer navigators, healthcare workers, and facility partners to achieve full biometric registration coverage for all clients in care and treatment.

Qualification and experience
  • 2-5 years of experience in KVP-focused HIV/AIDS community projects as well as 2-3 years in CTCs (Care and Treatment Centers).
  • Certified clinician or nurse officer with strong experience in HIV/AIDS programs, deep knowledge of HIV/AIDS care, and 2-5 years of experience working at CTCs.
  • A medical degree is a plus.
  • Proficient in MS Excel, MS Word, and MS PowerPoint.
  • A good team player with proven skills and experience in leading and coaching a team.
  • Ability to network and maintain professional relationships with the LGAs and other stakeholders at the Council level.
  • Excellent written, presentation, communication, and organizational skills in both English and Kiswahili
  • Good understanding of the council-level healthcare system · Flexible to work beyond normal working hours.
